No need to show example we all have experienced it.
Control often skips rule book, P Way improved after publication of Working TT, Train got full priority had free run, Train scheduled to run ahead delayed due any reason.
are some reasons.
Attached Old Working TT (Please don't ask for translation) gives fair idea how Loose time is given and how crossings are planned.

Rules are set, but most of them taken double extra care. Also many of this working TT are not practical. What i mean is the reason for allowance is due to some cause. IR should tried to minimize them , so that train can be speed up.
Controls ignores ?? Its says the validity of this book. We cannot defend on data which is created to safegaurd thenselfs !!

Thumb rule.
Paper record have validity over oral deeds.
When something goes wrong only these records are quoted.

Ya that's true. ANy ways as long as these issues are sorted out even if they make 160 kmph tracks it wont help. There many trains struck in slots and running like passenger train.

Making a train daily
(1) lead to convenience to people as they can straight forward plan their journey with out bothering about availability of train on a particular day,
(2) leads to better/ rake utilisation
(3) better utilisation of time slot
(4)
...
convenience to railway officials
(5) but give minimum political mileage.
On the other hand announcing a new weekly train gives more political weightage, In spite of above we find more and more new weekly/ biweekly trains announced than regularisation of existing trains.
For stoppage there is a policy.
(1) First make a grand announcement of a new superfast train with minimal stops and timing and derive political mileage.
(2) Then announce stops one after the other and get further political mileage.
(3) On some instances, same train is inaugurated again and again some times as special train and then regularisation.
This way people realise that it is their representative who is facilitating them with new trains and not the market demand for such train. This is IR!!
