To get a medical card, individuals need to bring their ID card, medical card, a verified pass declaration letter, photographs of themselves and their dependents, proof of birthdate (Aadhaar card or school certificate), and the mobile phone number to be used for registration. The camps will be held at various locations throughout the month of October and November, starting on October 3rd in Anand Nagar and ending on November 14th in Basti.
