I am a regular traveller..& travelling most of time in eastern sector by Rajdhani trains(Koltatta/Seyaldh/Ranchi/Bhubnashwar/Patna).
In every trip i have to alert Coach attendant to provide me clean bed rolls. Most of the time there reply is that... sir i placed the clean bed rolls on your seat...but unfortunately in 80% cases the bed roll were nicely wrapped not cleaned, then they search for clean one..when i threaten to lodge complain.
After providing the clean one almost 95% CA said that we have been provided the same unclean bed rolls by authorities but...

Second thing i have marked in 75% cases that the food supplied for lunch & dinner are tasteless and quantity is low, but due to embarrassment no one in coach raises question...
If some passenger like me (which is very few) ask the attendant of pantry car for the same, then he treat as take in granted.
Further if asked for complaint book then all the crew members from pantry car uses all the tricks for not allowing passengers to lodge the complain.
I think a lot of passengers like me face the same music..& looking for a platform to lodge their grievances.
